  <class>SmallMolecule</class>
<description>Zoledronate belongs to the bisphosphonates, a subclass of organic phosphonic acids and derivatives within the organic compounds. It is an exogenous solid with the formula C5H10N2O7P2 and an average molecular weight of 272.09 g/mol. This compound is used as a drug (PMC10692760) and a bone density conservation agent (PMC10692760), specifically within healthcare, pharmaceuticals, and veterinary products for the treatment of bone diseases. It is administered via intravenous exposure.

Zoledronate targets farnesyl pyrophosphate synthase (FDPS), geranylgeranyl pyrophosphate synthase (GGPS1), and matrix metalloproteinase-14 (MMP14). Its action on bone tissue is partly based on its affinity for hydroxyapatite in the bone mineral matrix. As a nitrogen-containing bisphosphonate, it acts as an analogue of isoprenoid diphosphate lipids to inhibit FPP synthase in the mevalonate pathway, which prevents the biosynthesis of isoprenoid lipids (FPP and GGPP) in osteoclasts.

Therapeutically, zoledronate is used to treat osteoporosis (PMC10560398, PMC12051078, PMC12068388, PMC12896522, PMC2971711, PMC4307702), hypercalcemia (PMC10547673, PMC12061736), multiple myeloma (PMC1779467, PMC3699134, PMC8951013), breast cancer (PMC1779467), prostate cancer (PMC1779467), lung cancer (PMC1779467), cancer (PMC3688704), and renal cell carcinoma (PMC11029699). It has demonstrated therapeutic effects on colorectal cancer (PMC5384426), arrhythmia (PMC12409151), pain (PMC5037694, PMC9645513), multiple myeloma (PMC3722725, PMC5740853), and breast cancer (PMC3480876, PMC4293349, PMC4723356). Reported adverse effects associated with the compound include fever (PMC2971711, PMC8533546) and acute renal failure (PMC13228822).</description>

<mechanism_of_toxicity>The action of zoledronate on bone tissue is based partly on its affinity for hydroxyapatite, which is part of the mineral matrix of bone. Zoledronate also targets farnesyl pyrophosphate (FPP) synthase. Nitrogen-containing bisphosphonates such as zoledronate appear to act as analogues of isoprenoid diphosphate lipids, thereby inhibiting FPP synthase, an enzyme in the mevalonate pathway. Inhibition of this enzyme in osteoclasts prevents the biosynthesis of isoprenoid lipids (FPP and GGPP) that are essential for the post-translational farnesylation and geranylgeranylation of small GTPase signalling proteins. This activity inhibits osteoclast activity and reduces bone resorption and turnover. In postmenopausal women, it reduces the elevated rate of bone turnover, leading to, on average, a net gain in bone mass.</mechanism_of_toxicity>
<metabolism>Zoledronate does not inhibit human P450 enzymes in vitro and does not undergo biotransformation in vivo.
Route of Elimination: In 64 patients with cancer and bone metastases, on average (&amp;#177; s.d.) 39 &amp;#177; 16% of the administered zoledronic acid dose was recovered in the urine within 24 hours, with only trace amounts of drug found in urine post-Day 2.
