Description
Methoxsalen belongs to the furanocoumarins, a subclass of coumarins and derivatives within the organic compounds. It is an exogenous solid with the formula C12H8O4 and an average molecular weight of 216.19 g/mol. This plant metabolite serves as a dermatologic drug, antineoplastic agent, photosensitizing agent, and cross-linking reagent. It is used as a treatment for psoriasis (PMC3524969 ). The compound is found in 13 recorded sources, including antipsoriatics, soap detergents, perfumes within detergents and soaps, non electric simulated smoking devices, the preparation of vinegar, food preservation, fermentation processes for beer, the preparation of wort, and electrical and electronic equipment such as electrodes, discharge lamps, conductive bodies or amplifiers, and electrically stimulated smoking devices. Exposure routes include topical, oral, and extracorporeal administration. Methoxsalen interacts with five recorded protein targets: Amine oxidase [flavin-containing] A (MAOA), Acetylcholinesterase (ACHE), Cytochrome P450 3A4 (CYP3A4), Cytochrome P450 2C9 (CYP2C9), and one other protein. Regarding health effects, the compound is associated with Fanconi anemia (PMC1241709 ).

Mechanism of ToxicityMethoxsalen is a cholinesterase or acetylcholinesterase (AChE) inhibitor. A cholinesterase inhibitor (or 'anticholinesterase') suppresses the action of acetylcholinesterase. Because of its essential function, chemicals that interfere with the action of acetylcholinesterase are potent neurotoxins, causing excessive salivation and eye-watering in low doses, followed by muscle spasms and ultimately death. Nerve gases and many substances used in insecticides have been shown to act by binding a serine in the active site of acetylcholine esterase, inhibiting the enzyme completely. Acetylcholine esterase breaks down the neurotransmitter acetylcholine, which is released at nerve and muscle junctions, in order to allow the muscle or organ to relax. The result of acetylcholine esterase inhibition is that acetylcholine builds up and continues to act so that any nerve impulses are continually transmitted and muscle contractions do not stop. Among the most common acetylcholinesterase inhibitors are phosphorus-based compounds, which are designed to bind to the active site of the enzyme. The structural requirements are a phosphorus atom bearing two lipophilic groups, a leaving group (such as a halide or thiocyanate), and a terminal oxygen. The mechanism of action many furocoumarins is based on their ability to form photoadducts with DNA and other cellular components such as RNA, proteins, and several proteins found in the membrane such as phospholipases A2 and C, Ca-dependent and cAMPdependent protein-kinase and epidermal growth factor. Furocoumarins intercalate between base pairs of DNA and after ultraviolet-A irradiation, giving cycloadducts. (5).
Carcinogenicity (IARC Classification)Methoxsalen plus UVA radiation is carcinogenic to humans (Group 1). (4)
Minimum Risk LevelNot Available
SymptomsHarmful if swallowed. May cause sensitisation by skin contact. (5)
TreatmentIf the compound has been ingested, rapid gastric lavage should be performed using 5% sodium bicarbonate. For skin contact, the skin should be washed with soap and water. If the compound has entered the eyes, they should be washed with large quantities of isotonic saline or water. In serious cases, atropine and/or pralidoxime should be administered. Anti-cholinergic drugs work to counteract the effects of excess acetylcholine and reactivate AChE. Atropine can be used as an antidote in conjunction with pralidoxime or other pyridinium oximes (such as trimedoxime or obidoxime), though the use of '-oximes' has been found to be of no benefit, or possibly harmful, in at least two meta-analyses. Atropine is a muscarinic antagonist, and thus blocks the action of acetylcholine peripherally.

Fanconi anemiaAssociated With? Not AvailableFanconi Anemia, also known as Fanconi hypoplastic anemia, Fanconi pancytopenia, and Fanconi panmyelopathy, is a rare condition that affects many parts of the body. About 90 percent of people with FA have impaired bone marrow function leading to aplastic anemia. More than half have physical abnormalities including light-colored skin or café-au-lait spots, malformed thumbs or forearms or other skeletal problems including short stature, malformed or absent kidneys or other urinary tract defects, gastrointestinal abnormalities, heart defects, small or abnormally shaped eyes, malformed ears, and hearing loss. They may also have abnormal genitalia or malformations of the reproductive system. Most males and about half affected females are infertile. Additional signs and symptoms of FA can include brain and spinal cord abnormalities. People with FA are at an increased risk of developing acute myeloid leukemia or tumors of the neck, head, skin, gastrointestinal system, or genital tract. FA is most often inherited in an autosomal recessive pattern, but in rare instances it is inherited in an X-linked recessive pattern. (NIH: GHR)PMC1241709
