Magnesium Sulfate belongs to the alkaline earth metal sulfates, a subclass of alkaline earth metal oxoanionic compounds within the inorganic compounds. This exogenous mixed metal/non-metal compound has the formula MgO4S and an average molecular weight of 120.37 g/mol. At room temperature, it exists as a solid. In medical contexts, it serves as an anticonvulsant, anaesthetic, analgesic, cardiovascular drug, anti-arrhythmia drug, and tocolytic agent (PMC11610491, PMC7553084). It is used as a treatment for eclampsia and has a therapeutic effect on ischemia (PMC8228929), though it is associated with hypotension (PMC10169116). Biologically, it acts as a calcium channel blocker (PMC11610491, PMC7553084). It targets several proteins, including the Voltage-dependent calcium channel gamma-1 subunit (CACNG1), Voltage-dependent calcium channel subunit alpha-2/delta-1 (CACNA2D1), Voltage-dependent L-type calcium channel subunit alpha-1C (CACNA1C), and Voltage-dependent L-type calcium channel subunit beta-1 (CACNB1). It also inhibits or blocks the Voltage-dependent L-type calcium channel subunit beta-4 (CACNB4), Voltage-dependent P/Q-type calcium channel subunit alpha-1A (CACNA1A), Voltage-dependent L-type calcium channel subunit alpha-1F (CACNA1F), and Voltage-dependent L-type calcium channel subunit beta-3 (CACNB3). Magnesium sulfate reduces striated muscle contractions and blocks peripheral neuromuscular transmission by reducing acetylcholine release at the myoneural junction, while inhibiting Ca2+ influx through dihydropyridine-sensitive, voltage-dependent channels. The compound is found in various sources, including electrical and electronic equipment, household cleaning and consumer products, building and construction materials, and food, food processing, and cookware. It is also present in healthcare and veterinary products such as minerals, IV solution additives, dermatological preparations, and drugs for constipation. Toxicity Profile
Mechanism of ToxicityMagnesium is the second most plentiful cation of the intracellular fluids. It is essential for the activity of many enzyme systems and plays an important role with regard to neurochemical transmission and muscular excitability. Magnesium sulfate reduces striated muscle contractions and blocks peripheral neuromuscular transmission by reducing acetylcholine release at the myoneural junction. Additionally, Magnesium inhibits Ca2+ influx through dihydropyridine-sensitive, voltage-dependent channels. This accounts for much of its relaxant action on vascular smooth muscle. The first warning of impending toxicity is loss of the patellar reflex at plasma concentrations between 3.5 and 5 mmol/L. Respiratory paralysis occurs at 5 to 6.5 mmol/L. Cardiac conduction is altered at greater than 7.5 mmol/L, and cardiac arrest can be expected when concentrations of magnesium exceed 12.5 mmol/L.

SymptomsAdverse reactions include hypotension, ECG changes, diarrhea, urinary retention, CNS depression and respiratory depression.
TreatmentEYES: irrigate opened eyes for several minutes under running water. INGESTION: do not induce vomiting. Rinse mouth with water (never give anything by mouth to an unconscious person). Seek immediate medical advice. SKIN: should be treated immediately by rinsing the affected parts in cold running water for at least 15 minutes, followed by thorough washing with soap and water. If necessary, the person should shower and change contaminated clothing and shoes, and then must seek medical attention. INHALATION: supply fresh air. If required provide artificial respiration.
