Description
Clindamycin is an exogenous organic compound with the formula C18H33ClN2O5S and an average molecular weight of 424.98 g/mol. It exists as a solid at room temperature. Clindamycin belongs to the amino acids, peptides, and analogues, a subclass of carboxylic acids and derivatives within the organic compounds. This compound acts as an inhibitor of the large ribosomal subunit protein uL1 (rplA). It is identified as a semisynthetic lincosamide antibiotic that inhibits bacterial protein synthesis by binding to bacterial 50S ribosomal subunits. Depending on the drug concentration and the specific organism, clindamycin may be bactericidal or bacteriostatic. It has largely replaced lincomycin due to an improved side effect profile. Clindamycin is found in or released from five recorded sources, including drinking water and water supply, as well as pharmaceuticals and veterinary products such as antibiotics, gynecologicals, anti-acne preparations, and macrolides, lincosamides and streptogramins. Recorded exposure routes for the compound include oral, intravenous, intramuscular, topical, vaginal, dental, and inhalation.

Mechanism of ToxicitySystemic/vaginal clindamycin inhibits protein synthesis of bacteria by binding to the 50S ribosomal subunits of the bacteria. Specifically, it binds primarily to the 23s RNA subunit. Topical clindamycin reduces free fatty acid concentrations on the skin and suppresses the growth of Propionibacterium acnes (Corynebacterium acnes) , an anaerobe found in sebaceous glands and follicles.
Carcinogenicity (IARC Classification)No indication of carcinogenicity to humans (not listed by IARC).
Minimum Risk LevelNot Available
SymptomsOrally and parenterally administered clindamycin has been associated with severe colitis (pseudomembranous colitis) which may result in patient death. Use of the topical formulation of clindamycin results in absorption of the antibiotic from the skin surface. Diarrhea, bloody diarrhea, and colitis (including pseudomembranous colitis) have been reported with the use of topical and systemic clindamycin.
