Description
Sodium nitrate (NNaO3) is an inorganic compound with an average molecular weight of 84.99 g/mol. It is a solid at room temperature and is exogenous in origin. Sodium nitrate belongs to the alkali metal nitrates, a subclass of alkali metal oxoanionic compounds within the inorganic compounds. The compound serves numerous industrial and biological roles. It is used as a fertilizer, preservative, corrosion inhibitor, bleaching agent, humectant, flavouring and nutrient, and as a soil amendment. Additionally, it functions as a degradant/impurity, oxidizing agent, biocide, dye, and chelating agent. It is utilized as a non-motive propellant (blowing agent) and an industrial additive (PMC9173806), as well as an industrial reducing agent (PMC8519076). Environmentally, it acts as a nutrient (PMC12655362), and biologically, it serves as an electron acceptor (PMC11366258, PMC13300293). Sodium nitrate is found in or released from 30 recorded sources, including food processing for meats, fish, and vinegar; household cleaning and consumer products such as bleaching agents and smoking requisites; building and construction materials like tents, canopies, and ceilings; and electrical and electronic equipment including amplifiers, antennas, and communication cables. Regarding health effects, sodium nitrate is used as a treatment for botulism (PMC8467560) and is associated with colorectal cancer (PMC10301060). Toxicity Profile
Mechanism of ToxicityNitrate's toxicity is a result of it's conversion to nitrite once in the body. Nitrite causes the autocatalytic oxidation of oxyhemoglobin to hydrogen peroxide and methemoglobin. This elevation of methemoglobin levels is a condition known as methemoglobinemia, and is characterized by tissue hypoxia, as methemoglobin cannot bind oxygen. (1, 5)

SymptomsNitrate and nitrite poisoning causes methemoglobinemia. Symptoms include cyanosis, cardiac dysrhythmias and circulatory failure, and progressive central nervous system (CNS) effects. CNS effects can range from mild dizziness and lethargy to coma and convulsions. (4)
TreatmentMethemoglobinemia can be treated with supplemental oxygen and methylene blue 1% solution administered intravenously slowly over five minutes followed by IV flush with normal saline. Methylene blue restores the iron in hemoglobin to its normal (reduced) oxygen-carrying state. (5)
